Metastatic colon cancer to the liver

Pain and fatigue

Reoccurrence of disease in 1998, "remarkable response" to I\' chemo with normalization of CT scan and chemistry one year ago; since then, tumor markers have been increasing and are out of proportion to patient's liver function tests, therefore received CPT-11 for tumor but bone marrow did not tolerate this drug requiring transfusions

Endometrial cancer, arthritis, pain

Pain from arthritis and cancer

Complaints of pain related to cancer and arthritis, following surgery, growth in uterus decreased in size, good recovery from surgery; no chemotherapy required

Surgical pathology revealed moderately differentiated adenocarcinoma (endometroid type) in endometrium, neoplasm confined to inner one-third of uterine wall, 2.4 mm maximum thickness, 1 mm maximum depth of invastion into inner one-third of myometrium, remaining endometium shows cystic atrophy and mild cystic glandular hyperplasia, follicular cyst of ovary and unremarkable fallopian rube tissue; endometrial biopsy in August 1999 revealed endometrial adenocarcinoma, grade 3 (11/10/99)

Small area of edema in right lobe significantly smaller than previous study on 1 /29/99; no pathologic-contrast enhancement is now identified; previous area of edema in left temporal lobe essentially resolved, no new pathology, mass effect, or midline shift observed, overall, resolution of edema in left temporal lobe since previous study with some continued edema or area of demyelination involving right frontal lobe at sight of previous rumor (9/28/99)

Nasopharyngeal carcinoma metastatic to bone

Constant pain in left ribs

History of nasopharyngeal

carcinoma, complains of pain on

left sight of back down to leg on

and off for over 4-5 years; constant

leg pain developed that restricted

patient movement (1/17/00);

coughing since 1991 that only happens in cold weather, complains of neck pain from 1.5 years ago

possibly due to work though x-rays

revealed negative findings; complains of left carache and had surgery performed behind left ear (12/99). now has constant pain behind left ear at surgical site for months; started to receive chemotherapy x 6 and radiation x 35 in April 1999 which together ended in August 1999; unable to eat so feeding tube inserted (5/99);

bead and neck surgery to remove cancerous growth (11 /99), now complains of pain on left side of ribs for the last 3 months along

with pain in the sternum, although

nasopharyngeal tumor size

decreased from 3 cm to 2 cm after

radiation and chemotherapy, cancer

spread to bones; life expectancy

estimated at 1 year

compared with prior study in 3/22/99; rib lesions, right femoral

sacral lesions; findings suspicious for metastatic disease (1/13/00); multiple pleural based left pulmonary masses present that have mildly increased in size along with increased left pleura! effusion from prior study on 4/7/99, mild increase in nodular pleura] thickening greatest in left lung

adenopathy mildly increased from prior study, stable 9 mm lesion in left lobe of liver which is likely benign
